Working with referring authorities
Each referral is subject to a full assessment to confirm the service can meet the individual’s needs and provide a safe, appropriate placement.
INFORMATION REQUIRED
To assess suitability we request:





Pre-placement visits to the service can be arranged on request.
Recent clinical summary
A summary of the individual's current clinical status and treatment history.
Risk assessment
Details of the individual's risk profile and safety needs within the community.
Medication details
A list of current prescriptions and any specific medication management requirements.
Care plan or discharge plan
Information about the ongoing care needs and goals for the transition.
Funding confirmation
Confirmation of funding arrangements for the potential placement.
ASSESSMENT PROCESS
Following receipt of documentation:

Referral reviewed
The management team reviews the referral details to ensure all clinical and safety information is complete and accurate.

Placement assessment
A detailed assessment is carried out to match the individual with the most suitable unit based on their specific needs and preferences.

Decision communicated
The decision is formally communicated to the referring authority, and the placement is confirmed for the individual.
